Job summary

Molina Healthcare is seeking a remote field-based Care Coordinator to support LTSS care management. You will conduct member assessments, develop and monitor care plans, and collaborate with interdisciplinary teams to coordinate services and improve outcomes for high-need members.

The role requires travel 25-40% and the ability to work independently in diverse settings, with strong communication skills and proficiency in Microsoft Office and online portals.

Qualifications

• At least 2 years of health care experience, LTSS exposure preferred.
• Clinical licensure and/or certification required only if state contract mandates it.
• Valid driver's license and reliable transportation for travel.
• Knowledge of community resources and ability to work independently.

Responsibilities

• Completes comprehensive member assessments within timelines, including home visits as required.
• Facilitates waiver enrollment/disenrollment and develops care plans with providers and family.
• Monitors care plan effectiveness and documents interventions and outcomes.
• Promotes integrated services across LTSS, behavioral health, and home/community resources.
• Assesses medical necessity, authorizes waiver services, and coordinates ICT meetings.
• Educates and supports members using motivational interviewing and Molina guidelines.
• Travel up to 25–40% as required by state contracts.
