Job summary

Cambia Health Solutions is seeking a Remote Care Management Nurse who can deliver care management services from home within Oregon, Washington, Idaho or Utah. This role involves case management, disease management and care coordination to improve quality and control costs.

Ideal candidates hold a nursing degree with an active RN license, 3+ years of clinical or care management experience, and licensure eligibility in the listed states.

Qualifications

• Nursing or health/human services degree (Associate's or Bachelor's minimum) or equivalent experience.
• Active, unrestricted licensure or certification allowing independent assessments — RN license required for medical care management.
• Must be eligible for licensure in Idaho, Oregon, Utah and Washington.

Responsibilities

• Conducts case management activities: assessment, planning, implementation, coordination, monitoring and evaluation.
• Apply clinical judgment to ensure compliance with medical policy and standards of care.
• Collaborate with physicians and other stakeholders to resolve issues and improve quality of care.
• Respond to inquiries professionally while protecting confidentiality of sensitive documents.
• Provide accurate documentation aligned with performance standards and timelines.
• Coordinate resources and prioritize assignments to meet goals and timelines.
• Monitor and evaluate effectiveness of care plans and adjust as needed.
