Job summary

Pathways Healthcare in Exeter, NH seeks a self-motivated Home Health & Hospice Liaison to develop outreach plans and increase referrals from hospitals, SNFs, ALFs, and physician offices. The role requires coordinating with care teams, assessing eligibility, and expanding patient admissions.

Responsibilities

• Identify, evaluate, and admit eligible patients into home health and hospice programs.

• Interpret clinical documentation and collaborate with providers to assess hospice eligibility.
• Communicate with physicians, nurses, and social workers to establish care plans.
• Maintain strong relationships with hospitals, nursing homes, physician practices, and discharge planners.
• Meet quotas to achieve budget goals.
• Serve as a trusted consultant and educator in care plan meetings and trainings.
• Track referral and admission metrics to ensure performance targets are met.
• Travel within the designated service area.

About the Role:

We’re looking for a self-motivated, articulate, and clinically experienced professional to join our team asHome Health & Hospice Liaison. In this role you will develop and execute strategic outreach plans to increase referrals and admissions from hospitals, skilled nursing facilities (SNFs), assisted living facilities (ALFs), and physician offices.

• Schedule : Monday - Friday

Responsibilities

• Identify, evaluate, and admit eligible patients into our home health and hospice programs.
• Facilitate conversations with home care teams, patients, and families.

• Personal or professional experience with hospice is a plus.
