Job summary

Agape Care Group is seeking a Clinical Liaison to coordinate patient referrals, build relationships with hospitals, and educate community and referral sources about hospice care. You will ensure continuity of care, clear communication, and proper documentation while collaborating with the care team.

The role involves working with leadership for professional development and advancement opportunities. Pay is determined by experience and locale and includes extensive benefits as part of our team.

Qualifications

• Graduate of an accredited school of nursing.
• 2 years of experience in a clinical care setting, hospice preferred.
• Reliable transportation and ability to lift at least 25 lbs.

Responsibilities

• Coordinate and generate new hospice business in existing and new accounts.
• Identify new markets and maintain existing service.
• Coordinate care for referred patients and ensure effective communication and documentation.
• Serve as liaison between the agency, hospitals, medical community and referral sources.
• Educate community and medical professionals about hospice services.
• Frequent work away from primary place of business as part of field duties.

Essential Functions

The Clinical Liaison is responsible for coordinating & generating new hospice business in both existing and new accounts, identifying new markets and maintaining existing service as well as coordinating care for referred patients. The Clinical Liaison's primary responsibility is serving as a liaison between the agency, hospitals, medical community and other referral sources. This position educates the community and the medical profession/referral sources regarding hospice services. This position involves daily interaction with patients, medical professionals, other referral sources, and the community to assure continuity of care and to coordinate appropriate communication and documentation. This position works closely with agency personnel to ensure that patient care is handled efficiently and effectively. The Clinical Liaison is regularly engaged away from the employee’s place of business in performing these duties.

Qualifications

• A heart to serve patients and families and a passion for providing the best possible care
• Education: Graduate of an accredited school of nursing
• Experience: 2 year of experience in a clinical care setting, hospice preferred
• Required: Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 25lbs and bear the weight of an average adult effectively.
