Job summary

CEDARCREEK seeks a Speech-Language Pathologist to evaluate patients within 24 hours of referral and develop treatment plans. The role includes supervising CFY/Speech Clinical Fellows and coordinating with the health team on patient progress and discharge planning.

Responsibilities include documenting evaluations and progress notes per state requirements and training families for post-discharge care. A Bachelor's in Speech-Language Pathology is required, with advanced degrees preferred.

• Ability to solve practical problems and follow written or schedule-based instructions.

Responsibilities

• Evaluate patients within 24 hours of referral and develop a treatment plan.
• Supervise CFY/Speech-Language Pathology Clinician Fellows in direct patient care.
• Record evaluations, progress notes, and discharge planning in accordance with policy and state requirements.
• Instruct families and nursing staff in maintenance and caregiver training for discharge from therapy services.
