Qualifications

• Experience in case management or clinical/therapeutic work (DDS)
• Four+ years full-time or equivalent part-time case management or clinical/therapeutic experience
• Substitutions: Bachelor's degree or higher may substitute for two years of required experience
• Willingness to travel as assigned
• Valid driver's license at appropriate class level for assignment

Responsibilities

• Ensure all components of the DDS ISP process or contracted services are completed for each recipient
• Respond to urgent requests and referrals to area programs and services
• Provide support and consultation to provider agencies and families
• Participate in provider meetings and Area Office Management Team activities
• Maintain automated data systems and manage related projects
• Contribute to MassCAP assessments to inform prioritization of services
