Job summary

Legacy Community Health is seeking a Manager to lead our Ryan White Case Management team in Houston. You will oversee a multidisciplinary group delivering medical and non-medical support to individuals living with HIV/AIDS, ensuring patient-centered, trauma-informed care.

Your leadership will drive quality improvement, policy alignment, and collaboration with partners to enhance access to services and health outcomes for diverse patients. Bilingual English/Spanish preferred.

Qualifications

• MSW from CSWE accredited program is required.
• Active LMSW license in Texas is required.
• Minimum three years paid post-degree social work experience.
• Two years supervisory experience and coaching能力.
• Experience with Ryan White program or HIV services preferred.

Responsibilities

• Provide direct supervision and manage daily activities of case management staff.
• Oversee Ryan White Case Management services for compliance and quality.
• Interpret and implement program policies aligned with organizational values.
• Monitor metrics tied to patient outcomes and drive improvement.
• Supervise recruitment, training, and evaluations of licensed/unlicensed staff.
• Promote trauma-informed HIV care and cultural humility within the team.
• Support professional development and collaborative work environment.
• Ensure comprehensive care plans considering diverse patient needs.
• Monitor service delivery for access, efficiency and quality.
• Provide guidance on complex cases respecting eligibility/docs standards.
• Lead quality management, audits, and data review.
• Maintain accurate documentation and reporting practices.
• Analyze program data to identify improvements and trends.
• Collaborate with teams and partners for coordinated care.
• Support billing and budgets aligned with funding requirements.
