Job summary

Priority Health Care is a Louisiana-based Federally Qualified Health Center serving individuals living with HIV, and seeks a Medical Case Manager to join the Ryan White Program and coordinate comprehensive healthcare services.

You will conduct eligibility screenings, develop individualized care plans, and coordinate referrals across medical and support services, providing direct advocacy to help clients navigate the HIV care continuum with empathy and professionalism.

Qualifications

• Must have licensure as a social worker or nursing licensure.

Responsibilities

• Screen clients for Ryan White program eligibility and maintain documentation.
• Conduct comprehensive assessments of medical, psychosocial, social, and financial needs.
• Develop individualized care plans with providers and community partners.
• Provide referrals to medical providers, mental health, substance use treatment, housing, food, and transportation.
• Follow up on referrals to ensure access to services and achievement of care plan goals.
• Provide advocacy and crisis support in clinic, community, or home settings.
• Counsel clients on HIV disease management, medication adherence, transmission prevention, and harm reduction.
• Document assessments, care plans, and client interactions in the EHR per HIPAA and Ryan White standards.
• Build relationships with community organizations to connect clients with HIV and support services.

Job description

Priority Health Care is seeking a compassionate Medical Case Manager to join our Ryan White Program and coordinate comprehensive healthcare services for clients living with HIV.

You’ll conduct eligibility screenings, develop individualized care plans, coordinate referrals across medical and support services, and provide direct advocacy to help clients navigate the HIV care continuum. Your ability to connect clients with services, understand HIV disease and treatment, and build trust with diverse populations is essential to our mission.

Responsibilities:

• Screen clients for initial and annual Ryan White program eligibility; maintain documentation and recertification records
• Conduct comprehensive assessments of medical, psychosocial, social, and financial needs
• Develop individualized care plans in collaboration with providers and community partners
• Provide referrals to medical providers, specialists, mental health, substance use treatment, housing, food, and transportation services
• Follow up on referrals to ensure clients access services and achieve care plan goals
• Provide advocacy and crisis support in clinic, community, or home settings
• Counsel clients on HIV disease management, medication adherence, transmission prevention, and harm reduction
• Document all assessments, care plans, and client interactions in the EHR system per HIPAA and Ryan White standards
• Ability to work independently with minimum supervision
• Build relationships with community organizations; connect clients to HIV and support services

Minimum Qualifications:

Applicants must meet one of the following qualification pathways:

• Licensed Social Worker: Current licensure as an RSW, CSW, LMSW, or LCSW and graduation from a program accredited by the Council on Social Work Education;
• Licensed Nurse: Current Louisiana licensure as a registered nurse (RN) or licensed practical nurse (LPN);
• Human Services Degree and Experience: A bachelor’s degree in a qualifying human services-related field and one year of post-degree experience providing direct services to the HIV target population. A master’s degree is preferred;
• Liberal Arts or General Studies Degree and Experience: A bachelor’s degree in liberal arts or general studies, including at least 16 credit hours in a qualifying human services-related field, and one year of post-degree experience providing direct services to the HIV target population

Additional Preferred Qualifications:

• Knowledge of HIV disease, treatment options, medication adherence, and prevention strategies
• Knowledge of healthcare and community resources, insurance programs, and benefits
• Proficiency with EHR systems and case management software
• Excellent communication and problem-solving skills; ability to build trust with diverse populations
• Bilingual ability in Spanish or Vietnamese is a plus
• Completion of HIV-specific training within 6 months of hire
