Job summary

Qualifications

• Bachelor’s degree in Physical Therapy and a current Ohio PT license.
• Masters/Doctorate preferred; CPR certification may be required by licensure.
• Proficient with Net Health and Point Click Care for documentation.

Responsibilities

• Evaluate residents to determine PT needs and rehabilitation potential.
• Develop and update individualized treatment plans to improve mobility and function.
• Supervise PT Assistants and coordinate with physicians and nursing staff.
• Document timely and accurately in medical records per policy and payer rules.
• Educate residents and families on home programs, safety, and fall prevention.
• Facilitate ordering of durable medical equipment to support independence.
• Participate in care conferences and quality improvement initiatives.
