Job summary

PACS Group is seeking a Staff Physical Therapist to evaluate residents, develop individualized treatment plans, and advance mobility and independence in partnership with physicians and the health team.

You will supervise PT assistants, train staff, ensure compliant documentation, and educate residents and families on goals and home exercise programs. Licensed PT in the state is required; CPR may be required.

Qualifications

  • Bachelor's degree in Physical Therapy; Master’s or Doctorate preferred.
  • Licensed Physical Therapist in the state; maintain license.
  • CPR certification if required by licensure.
  • Proficient with Net Health and PointClickCare.
  • Ability to communicate with residents, families and healthcare team.

Responsibilities

  • Evaluate residents to determine PT needs and rehabilitation potential.
  • Develop and update individualized treatment plans.
  • Supervise PT assistants and train staff.
  • Communicate progress with physicians and families.
  • Maintain compliant documentation per regulations.
  • Ensure safe treatment environment and infection control.
  • Assist with discharge planning and home exercise programs.
  • Recommend durable medical equipment as appropriate.
