Job summary

PACS is seeking a Staff Physical Therapist to evaluate and treat residents, develop individualized treatment plans, and document care in the medical records. You will supervise PT assistants, coordinate with physicians and families, and contribute to quality assurance initiatives.

Candidates must be licensed in Ohio and hold a degree in Physical Therapy; proficiency with Net Health and Point Click Care is preferred. Strong communication and teamwork are essential for success in this role.

Qualifications

• Licensed Physical Therapist in the state of Ohio.
• Bachelor’s degree in Physical Therapy; advanced degree preferred (MS/PhD or DPT).
• Proficient with electronic medical records and rehab documentation.

Responsibilities

• Evaluate residents to determine PT needs and rehab potential.
• Develop, implement, and update individualized treatment plans.
• Supervise PT assistants and staff in resident care.
• Communicate progress with physicians, families, and interdisciplinary team.
• Maintain timely and compliant documentation per policies and payer requirements.
