Job summary

PACS is seeking a Staff Physical Therapist to evaluate and treat residents, communicate with families and physicians, and maintain documentation in medical records. The role includes in-services and training for facility staff on an ongoing basis.

The candidate will supervise PT assistants, develop treatment plans, and ensure compliant documentation while maintaining a safe, infection-controlled environment. Licensure and continuing education are required.

Qualifications

• Bachelor’s degree in Physical Therapy with licensure in the state.
• Masters or Doctorate in Physical Therapy preferred.
• Proficiency with Net Health and Point Click Care software.

Responsibilities

• Evaluate residents to determine PT needs and rehabilitation potential.
• Develop and update individualized treatment plans to improve mobility and function.
• Supervise PT assistants and ensure adherence to state practice act.
• Communicate progress with physicians, nursing staff, residents, and families.
• Complete timely, accurate documentation per policy and payer requirements.
• Educate residents and families on home exercise programs and fall prevention.
