Job summary

GRANDVIEW in Tennessee seeks a Staff Physical Therapist to evaluate residents, plan and implement treatments, and document progress in medical records. This role also trains facility staff and coordinates with families and physicians to achieve discharge goals.

You will supervise PT assistants, participate in care conferences, and ensure safety and infection control in daily practice. State licensure and CE requirements apply, CPR if required.

Qualifications

• Bachelor's degree in Physical Therapy with licensure in the state.
• Master's or Doctorate in Physical Therapy preferred.
• Proficient in Net Health and Point Click Care documentation.
• Ability to read technical procedures and policies.

Responsibilities

• Evaluate residents to determine PT needs, develop treatment plans.
• Treat residents to improve mobility, strength, balance and independence.
• Supervise PT assistants and nursing staff in resident care.
• Maintain accurate documentation per facility and payer requirements.
• Educate residents and families on goals, home programs, and safety.
• Participate in care conferences, Utilization Review and rehab meetings.
