Qualifications

• Licensed to practice Physical Therapy in the state
• Continuing education requirements must be met
• Ability to input and access records in Net Health/PointClickCare

Responsabilités

• Evaluate residents to determine PT needs and rehabilitation potential.
• Develop and update individualized treatment plans to improve mobility and independence.
• Supervise Physical Therapy Assistants and students in resident care.
• Communicate progress and discharge plans with physicians, nursing staff, families.
• Maintain timely, accurate documentation in compliance with policy and payer requirements.
