Job summary

Woodside Post Acute is seeking a Staff Physical Therapist to evaluate residents, design and update treatment plans, and support rehabilitation goals. You will supervise PT assistants and collaborate with physicians and families to ensure optimal mobility and safety.

Responsibilities include accurate documentation, progress reporting, and contributing to quality assurance initiatives in a Post-Acute setting. A current PT license and licensure maintenance are required.

Qualifications

• Licensed to practice physical therapy in the state.
• Bachelor’s degree in Physical Therapy; advanced degrees preferred.
• Proficient with PT documentation and medical record keeping.

Responsibilities

• Assess residents to determine PT needs and rehabilitation potential.
• Develop and update individualized treatment plans to improve mobility and independence.
• Supervise PT assistants and educate staff on therapy protocols.
• Document progress and discharge planning in medical records.
