Job summary

West Valley Post Acute is seeking a Staff Physical Therapist to evaluate residents, create and adjust treatment plans, and lead rehabilitation programs. You will collaborate with physicians, nurses, and families to ensure goals are met and discharge planning proceeds smoothly.

The role requires a state license, a PT degree, and the ability to educate staff and families on mobility and safety strategies. Independent clinical judgment is essential.

Qualifications

• Bachelor’s degree in Physical Therapy; Masters/Doctorate preferred.
• Licensed as a Physical Therapist in the state; continuing education as required.
• Proficient in Net Health and Point Click Care.
• CPR certification, if required by licensure.

Responsibilities

• Evaluate residents to determine PT needs and rehabilitation potential.
• Develop and update individualized treatment plans to improve mobility and independence.
• Supervise Physical Therapy Assistants and educate staff as needed.
• Communicate with physicians, nursing staff, families about progress and discharge goals.
• Complete timely, accurate documentation per policy and payer requirements.
• Maintain safe environment and participate in quality improvement activities.
• Provide resident and family education on home programs and safety.
