Job summary

Center for Human Development (CHD) in West Springfield, MA is seeking an Outreach Program Manager to lead and support adults with developmental disabilities living in the community. This role combines direct impact, team leadership, and program oversight to help individuals achieve their goals and live independently.

You will supervise Outreach staff, coordinate assessments and plans, ensure appropriate supports, and build partnerships with families and providers.

Qualifications

• Bachelor's degree in Human Services or a related field is preferred.
• Experience in developmental disabilities, behavioral health, human services, or a related field.
• Prior supervisory or leadership experience is preferred.
• Strong communication, problem-solving, and organizational skills are essential.
• Ability to build relationships with participants, families, community partners, and staff.
• Valid driver's license and reliable transportation.
• Ability to successfully complete required background and driving record checks.

Responsibilities

• Providing leadership, coaching, training, and supervision to Outreach staff.
• Supporting participant assessments, service planning, and goal development.
• Ensuring individuals receive appropriate medical, behavioral, and community-based supports.
• Promoting skill development, independence, self-advocacy, and community inclusion.
• Managing staff schedules and service delivery coverage.
• Monitoring documentation, compliance, billing, and program quality standards.
• Building collaborative partnerships with families, DDS, healthcare providers, and community resources.
• Participating in after-hours on-call support as part of a leadership team rotation.
• Responding to participant and staffing needs in a fast-paced, dynamic environment.
