Job summary

Center for Human Development (CHD) is seeking an Outreach Program Manager to lead staff, support individuals with developmental disabilities, and oversee service planning in a community setting. Qualifications

• Bachelors degree preferred in Human Services or related field.
• Experience in developmental disabilities, behavioral health, or human services.
• Prior supervisory or leadership experience preferred.
• Strong communication, problem-solving, and organizational skills.
• Ability to build relationships with participants, families, community partners, and staff.
• Valid drivers license and reliable transportation.
• Ability to complete required background and driving record checks.

Responsibilities

• Provide leadership, coaching, training, and supervision to Outreach staff.
• Support participant assessments, service planning, and goal development.
• Ensure individuals receive medical, behavioral, and community supports.
• Promote skill development, independence, and self-advocacy.
• Manage staff schedules and service delivery coverage.
• Monitor documentation, billing, and program quality standards.
• Build partnerships with families, DDS, healthcare providers, and community resources.
• Participate in after-hours on-call leadership rotation.
• Respond to participant and staffing needs in a fast-paced environment.

This position combines direct impact, team leadership, and program oversight to help individuals achieve their goals and live their fullest lives.

What You'll Do

• Providing leadership, coaching, training, and supervision to Outreach staff
• Supporting participant assessments, service planning, and goal development
• Ensuring individuals receive appropriate medical, behavioral, and community-based supports
• Promoting skill development, independence, self-advocacy, and community inclusion
• Managing staff schedules and service delivery coverage
• Monitoring documentation, compliance, billing, and program quality standards
• Building collaborative partnerships with families, DDS, healthcare providers, and community resources
• Participating in after-hours on-call support as part of a leadership team rotation
• Responding to participant and staffing needs in a fast-paced, dynamic environment
