Job summary

Center for Human Development (CHD) Care seeks an Outreach Program Manager to lead teams supporting adults with developmental disabilities living independently in the community. You will supervise staff, assist with assessments and service planning, and ensure access to medical, behavioral, and community supports.

Qualifications

• Bachelor's degree in Human Services or a related field preferred.
• Experience in developmental disabilities, behavioral health, human services, or a related field.
• Prior supervisory or leadership experience preferred.
• Strong communication, problem-solving, and organizational skills.
• Ability to build relationships with participants, families, community partners, and staff.
• Valid driver's license and reliable transportation.
• Ability to successfully complete required background and driving record checks.

Responsibilities

• Providing leadership, coaching, training, and supervision to Outreach staff.

• Participating in after-hours on-call support as part of a leadership team rotation.
• Responding to participant and staffing needs in a fast-paced, dynamic environment.
