Job summary

Memorial Hermann Health System is seeking a senior hospital outpatient coder to assign accurate ICD-10-CM and CPT diagnoses and procedures for emergency, clinics and diagnostics. You will review records, query physicians for clarity, and ensure compliance with AHIMA guidelines.

The role requires RHIT/RHIA or AHIMA/AAPC certification, 6+ years of coding experience in outpatient settings, and strong analytical and communication skills.

Qualifications

• Six (6) years of hospital outpatient or ambulatory coding experience.
• Experience coding in a level 1 trauma facility/academic teaching facility preferred.
• Effective oral and written communication skills.
• Strong knowledge of ICD-10-CM and/or CPT coding.
• Analytical skills to interpret health records and assign codes.
• Knowledge of human anatomy, physiology, medical terminology and surgical terminology.
• Critical thinking, judgment and decision-making skills.
• Knowledge of coding compliance policies and guidelines.

Responsibilities

• Review medical records to identify diagnoses/procedures requiring code assignment.
• Code diagnoses and procedures using ICD-10 conventions for reimbursement and compliance.
• Query physicians when documentation is unclear or ambiguous for coding.
• Stay current with coding guidelines and reimbursement reporting standards.
• Assign ICD-10-CM, modifiers and HCPCS codes to outpatient records.
• Maintain 92%–95% adherence in ICD-10-CM and HCPCS coding with productivity targets.
• Mentor and train newly hired coders and provide ongoing training.
• Assist Coding Manager with special coding tasks and unbilled issues.
• Serve as a resource for coding questions and support the team.
