Job summary

Columbia Post Acute in is seeking an Occupational Therapist to evaluate patients within 24 hours, develop treatment plans, and supervise COTAs in therapy activities. The role includes family training, discharge planning, and interprofessional communication to ensure optimal patient outcomes.

The candidate should hold a licensed OT in the state, with proficiency in Casamba and Point Click Care, and be able to document and coordinate care in alignment with board and payer requirements.

Qualifications

• Bachelor's degree in Occupational Therapy required.
• Licensure as an Occupational Therapist in the state. Continuing education as required by the state.
• Proficiency with Casamba and Point Click Care for documentation and data input.

Responsibilities

• Evaluate patients within 24 hours of referral.
• Develop and obtain approval for the treatment plan from the referring physician.
• Treat patients per the physician treatment plan and monitor progress.
• Supervise COTA, aides, and students in therapy activities.
• Assist nursing with Restorative Aide training and discharge planning.
• Document evaluations, progress notes, and discharge summaries per policy.
