Job summary

Lakeview Post Acute is seeking an Occupational Therapy Assistant to evaluate patients within 24 hours or one business day of referral and to develop treatment plans aligned with physician orders. The role includes supervising Certified Occupational Therapy Assistants and aides, documenting evaluations and progress notes, and participating in care conferences and discharge planning.

Strong computer skills (Casamba, Point Click Care) are preferred.

Qualifications

• Bachelor's degree in Occupational Therapy; Masters or Doctorate preferred.

Responsibilities

• Evaluate patients within 24 hours, or 1 business day of physician referral.
• Develop treatment plans and obtain approval from referring physician.
• Treat patients per the physician treatment plan.
• Supervise Occupational Therapy Assistants and aides in direct patient care.
• Record evaluations, daily treatment notes, and progress notes.
• Participate in discharge planning and caregiver training for home programs.
