Job summary

Open Sky Community Services in Worcester, MA, is seeking a Navigator Supervisor to provide direct service, clinical and administrative support, and operational oversight within Community Services programs. Qualifications

• High School Diploma, GED or equivalent required.
• Valid driver’s license with reliable transportation.
• Minimum of at least 3 years’ experience working with or lived experience with homelessness, justice involvement, adults with serious and persistent mental illness, substance use disorders, and underserved populations, required.
• Demonstrated experience providing leadership, oversight, or informal supervision, required.
• Bachelor’s degree in social work, psychology, or a related field preferred.

Responsibilities

• Carry a caseload of approximately 10–15 members enrolled in Community Services Programs, Recovery Support Navigation, Behavioral Health Justice Involved and/or Community Support Program for People Experiencing Chronic Homelessness.
• Engage with members as they enter the treatment system, including prior to release and support the members in gaining access to treatment services, housing opportunities and community resources.
• Provide mentoring, training, coaching, and support to a small number of Community Services Navigators, as assigned.
• Demonstrate respect for all members regardless of ability status, culture, sexual orientation, gender identity, religion, or ethnicity.
• Complete all Assessments and Support Plans according to prescribed timelines and specifications.
• Work collaboratively with medical and behavioral health providers to obtain any active comprehensive care plans that the member may have.
• Develop a Support Plan with members which guides services in a way that is consistent and supportive of other existing comprehensive care plans.
• Implement interventions as identified in the Support Plan and document Member progress to maintain a permanent record of activity and services provided.
• Act as a liaison between members, community agencies, and families to coordinate treatment, including employment and education entities, Care Coordination entities, Emergency Service Providers, outpatient clinics, and others.
• Provide temporary assistance with transportation to essential medical and behavioral health appointments while transitioning to community-based transportation resources.
• Provide and coordinate crisis response supports to members as necessary.
• Acquire and maintain all necessary training and certifications.
• Complete all required documentation in a timely manner.
• Attend and participate in meetings/trainings as assigned.
• Participate in supervision, team and other assigned meetings.
• Communicate all potential risk-related and other pertinent information to program leadership according to established program and Agency procedures.
• Participate in the On-Call rotation.
• Perform other duties as assigned.
