Job summary

Open Sky Community Services Inc. is looking for a Community Services Navigator to provide case management and outreach support to individuals in need.

This role is vital for those with mental health disorders, particularly those at high risk for 24-hour treatment settings. You will develop support plans, coordinate with various community services, and assist members with transportation to essential medical appointments.

Qualifications

• At least 2 years’ experience with homelessness or mental illness.
• Valid Driver’s License with acceptable driving history.
• Experience with underserved populations required.

Responsibilities

• Develop Support Plans for members of the program.
• Complete Assessments and Support Plans on time.
• Act as a liaison between members and community agencies.
• Coordinate treatment with medical and behavioral health providers.
• Provide temporary transportation assistance for appointments.
• Participate in On-Call rotation.
