Job summary

Open Sky Community Services, Inc. seeks a Community Services Navigator to deliver case management and outreach within Mass Health funded programs.

You will support members with high risk for admission to 24-hour treatment settings, coordinating services across providers and community agencies. Responsibilities include developing support plans, completing assessments, liaising with families and employment/education partners, and assisting with transportation while transitioning to community

Qualifications

• Strong communication, documentation, and organizational skills required.
• Experience or lived experience with homelessness, justice involvement, or chronic mental health challenges is valued.
• Valid driver's license and reliable transportation required.

Responsibilities

• Develop a Support Plan with the member to guide services and align with other care plans.
• Complete Assessments and Support Plans within prescribed timelines and specs.
• Act as liaison between members, community agencies, families, and care coordinators to coordinate treatment.
• Collaborate with medical and behavioral health providers to obtain active care plans and services.
• Provide temporary transportation assistance to medical and behavioral health appointments during transition to community resources.
• Participate in an On-Call rotation.
