Job summary

Open Sky Community Services in Worcester, MA, seeks a Community Services Navigator to provide case management and outreach for members enrolled in Community Services Programs, Recovery Support Navigation, Behavioral Health Justice Involved, and related supports.

The role requires a High School Diploma, a valid driver's license, and at least 2 years' experience with homelessness or justice-involved populations, plus strong communication and coordination skills.

Qualifications

• High School Diploma, GED or equivalent.
• Valid Driver's License and reliable transportation.
• Minimum 2 years of experience with homelessness, justice involvement, adults with serious mental illness, substance use disorders, and underserved populations.
• Excellent communication, documentation, time management and organizational skills.

Responsibilities

• Develop a Support Plan with the member and complete Assessments and Plans on schedule.
• Act as liaison between members, agencies, families to coordinate treatment and care.
• Provide transportation assistance to medical and behavioral health appointments during transition to community resources.
• Participate in On-Call rotation.
• Perform outreach duties as needed.
