PIH Health is seeking an Registered Nurse (RN) for outpatient hospice, working 8:30am–5pm in Whittier, CA. The RN will perform patient assessments, develop care plans, and educate patients and families about hospice services.

The role requires a CA RN license, BLS, and the ability to coordinate with a multidisciplinary team to ensure timely care and compassionate support for terminally ill patients and their families.

Qualifications

• Excellent written and verbal communication.
• Interpersonal skills to support patients and families.
• Clinical knowledge for terminal care needs.
• Strong organizational and basic computer skills.
• Demonstrated leadership abilities.
• Patient assessment expertise.
• Ability to educate patients and families about hospice care.
• Knowledge of Hospice Condition of Participation.
• Valid California driver’s license.
• Current BLS/CPR certification.

Responsabilités

• Provide direct nursing care including assessment, plan of care, education and evaluation for hospice patients.
• Coordinate care delivery with other care team members to ensure smooth patient flow.
• Offer supportive care to patients’ families and significant others.
