Job summary

Anthem Hospice is seeking a dedicated Registered Nurse to join our California hospice team. You will coordinate and deliver individualized end-of-life care in patients' homes and other settings, working closely with families, physicians, and the interdisciplinary team.

The role emphasizes symptom management, patient education, and compassionate support to ensure high-quality care throughout the hospice journey.

Qualifications

• Current California RN license to practice as a registered nurse.
• Experience with hospice or end-of-life care preferred.
• Ability to work with multidisciplinary teams and provide patient education.

Responsibilities

• Coordinate and deliver individualized hospice nursing in patients' homes and care settings.
• Assess patient needs, provide nursing care per physician orders and plan of care.
• Educate patients and families on disease progression and self-care.
• Document care, monitor symptoms, and adjust plans as patient condition changes.
• Collaborate with physicians, social workers, and other team members to ensure quality care.
