Job summary

PIH Health in Whittier, CA is seeking a Registered Nurse (RN) for Hospice Outpatient to join our full-time team of care providers. You will deliver direct nursing care for terminally ill patients certified for Hospice, including assessment, planning, education, and evaluation, while coordinating care with a multidisciplinary team.

The ideal candidate will hold a current BLS card, a valid California driver's license with automobile insurance, and have Hospice or Medicare Hospice experience

Qualifications

• Provides direct nursing care including assessment, plan of care, education, and evaluation for Hospice patients.
• Coordinates care with other team members and disciplines.
• Responds to patient, family and caregiver needs after hours, holidays and weekends.
• Requires Current Basic Life Support (BLS) certification and valid CA driver's license with automobile insurance.
• Public Health Nurse and Certified Hospice and Palliative Nurse are preferred.

Responsibilities

• Provides direct nursing care including assessment, plan of care, education, and evaluation.
• Coordinates care with the care team and disciplines to ensure patient flow.
• Responds to patient and family needs after hours, weekends and holidays.
