Job summary

PIH Health in Whittier, CA is seeking a Registered Nurse (RN) for Hospice Outpatient to join our full-time team of care providers. You will deliver direct nursing care for terminally ill patients certified for Hospice, including assessment, planning, education, and evaluation, while coordinating care with a multidisciplinary team.

The ideal candidate will hold a current BLS card, a valid California driver's license with automobile insurance, and have Hospice or Medicare Hospice experience

Qualifications

• Provides direct nursing care including assessment, plan of care, education, and evaluation for Hospice patients.
• Coordinates care with other team members and disciplines.
• Responds to patient, family and caregiver needs after hours, holidays and weekends.
• Requires Current Basic Life Support (BLS) certification and valid CA driver's license with automobile insurance.
• Public Health Nurse and Certified Hospice and Palliative Nurse are preferred.

Registered Nurse (RN) Hospice Outpatient, Full-time, 3pm - 11:30pm

Provides direct nursing care including assessment, plan of care, education, and evaluation for patients certified as having a terminal illness who have elected the Hospice benefit. Performs patient duties necessary to maintain the flow of patient care as well as coordinates the delivery of care provided by other care team members and disciplines. The Hospice Registered Nurse provides supportive care to the patient, family and/or significant others and is responsible for responding to all of their needs after business hours, holidays, and weekends.
