Job summary

St. Croix Hospice is seeking a Registered Nurse Case Manager to lead each patient’s hospice care team and manage their Plan of Care in collaboration with the Hospice Medical Director, patient, family, and facility staff.

Responsibilities include day-to-day patient care, administering medications, documenting care, coordinating with the medical team, and providing emotional support to patients and families. On-call duties may include evenings and weekends.

Qualifications

• Must hold a current RN license in state of practice.
• Graduate of professional nursing program and licensed in state(s) of practice.

Responsibilities

• Lead patient care team and manage individual Plan of Care.
• Administer medications and treatments as prescribed.
• Maintain accurate clinical documentation and HIPAA compliance.
• Coordinate care with physicians, interdisciplinary teams, and community partners.
• Provide emotional support to patients and families.
• Participate in on-call visits as required and attend meetings.
