Job summary

Mercy House in Santa Ana, CA is seeking an ECM Lead Care Manager to coordinate comprehensive medical and social services for people experiencing homelessness. The role focuses on team-based, patient-centered care and collaboration with CalAIM Community Supports staff to improve health outcomes.

You will manage 17-25 clients, conduct health assessments, and help navigate Medicaid and LTSS services. Requires a Bachelor’s in Social Work or related field and experience with homelessness or mental

Qualifications

• Bachelor’s Degree in Social Work (or related field) preferred.
• 2+ years of direct experience with homeless, mental health, or substance use issues welcomed.

Responsibilities

• Maintain a caseload of 17-25 individuals and conduct Health Needs Assessments and Care Plans.
• Engage and enroll ECM members in services and address barriers to care.
• Coordinate transportation and accompany members to critical appointments as needed.
• Advocate for members with healthcare professionals and coordinate with hospitals.
