Job summary

Evergreen Treatment Services is seeking a Case Manager for the LEAD program in Rainier Beach. You will engage individuals suspected of low-level offenses, provide intensive case management, and coordinate with partners to help improve housing, health, and stabilization outcomes.

Responsibilities include outreach, service coordination, and advocacy within the criminal justice system. A strong commitment to harm reduction and community safety is essential.

Qualifications

• High school diploma or equivalent required; further education desirable.
• Understanding of harm reduction and services for people experiencing homelessness.
• Ability to advocate within the criminal justice system and court settings.
• Strong interpersonal, organizational, and written communication skills.

Responsibilities

• Provide outreach and intensive case management to assigned participants.
• Engage participants on the street and with service providers to establish trust.
• Assess needs and connect participants to medical, mental health, and housing resources.
• Develop individualized service plans and coordinate with LEAD partners.
