Job summary

Evergreen Treatment Services is seeking a Community Justice (L.E.A.D.) Case Manager to join the REACH team in Seattle. You will provide intensive case management for a caseload of about 25–30 participants, focusing on engagement, outreach, housing assistance, and linkages to health and social services.

You will collaborate with LEAD partners, service providers, housing providers, and other community organizations to improve community health and safety.

Qualifications

• High school diploma or equivalent required. Further education/training is desirable.
• Ability to respectfully engage and develop a working alliance with the people we are serving is essential.
• Understanding of harm reduction and passion for serving individuals experiencing homelessness and co-occurring disorders required.

Responsibilities

• Provide outreach and intensive case management to participants.
• Engage participants on the street and at service providers to establish relationships and offer services.
• Assess severity of chemical dependency and housing status to determine needs for medical and mental health services.
• Assist participants in accessing funding programs and housing.
• Develop individualized Service Plans with participants and coordinate with LEAD partners and community organizations.
• Develop relationships with housing resources and advocate for systemic changes.
