Job summary

Passages to Recovery is seeking a field-based LEAD Case Manager to engage participants with repeated law enforcement contacts and provide intensive, client-centered case management. The role emphasizes trauma-informed, non-coercive harm-reduction approaches and strong collaboration with police, DHS, ACHD, and community providers.

You will support participants in housing, benefits, health care, and social services, while maintaining long-term engagement and culturally competent practices.

Qualifications

• Commitment to social and racial justice.
• Experience serving populations in poverty, with behavioral health needs or justice system involvement.
• Ability to build trust with police, providers and government stakeholders.
• Experience advocating for Participants in court and in writing.
• Street outreach experience delivering services to LEAD target populations.

Responsibilities

• Provide on-call coverage and rapid referrals to referral locations across Allegheny County.
• Engage Participants, obtain consent, and complete initial screenings.
• Assess needs including housing, health, and social services.
• Develop individualized service plans with participants and connect to Harm Reduction services.
• Transport Participants to appointments and services as needed.
• Advocate for Participants with other providers and the criminal legal system.
• Maintain Participant files and use required database tools.
• Foster collaborative relationships with LEAD partners and stakeholders.

Registered nurse with 2 years in public health
