Job summary

County of Los Angeles is recruiting for a supervisory role overseeing Senior Community Health Workers and Community Health Workers, directing client services and program operations.

Qualifications

• Option I: Three years of experience at the level of Senior Community Health Worker or similar.
• Option II: Five years of experience at the level of a Community Health Worker.

Responsibilities

• Supervises the work of a team of Senior Community Health Workers and Community Health Workers in providing direct health, mental health, or social services to clients and patients.
• Assigns service referrals to assist clients in obtaining food stamps, housing, medical services, mental health services, substance use care and other supportive services.
• Leads and facilitates development of independent living skills for clients through support groups on topics like housekeeping, cooking, budgeting, and transportation.
• Recommends improvements to work methods, policies and procedures, and oversees their implementation.
• Identifies gaps in community programs and recommends creative solutions.
• Advocates for client access to County and community resources and services.
• Plans and evaluates community health programs and supportive services in line with laws and regulations.
