Children’s Hospital Los Angeles in Los Angeles, CA, seeks a Supervisor, Community Health Services to oversee Enhanced Care Management and a team of community health workers. The Supervisor, Community Health Services will oversee the day‑to‑day activities of a team of community health workers focused on Enhanced Care Management. The role involves providing direct support in managing patient challenges, collaborating with management to build the team, and ensuring the delivery of high‑quality patient care. The incumbent will work closely with the Manager to coordinate personnel administration, provide administrative support to the administrative leadership, and assist in leading staff meetings. Additionally, the Supervisor will oversee Community Health Worker activity, attend leadership meetings, and demonstrate a commitment to a patient and family‑centered model of care.
Bachelor’s degree in Public Health, Health Administration, Child Development, Social Work, or similar discipline preferred or equivalent experience preferred.
