Job summary

Children’s Hospital Los Angeles in Los Angeles, CA, seeks a Supervisor, Community Health Services to oversee Enhanced Care Management and a team of community health workers. You will coordinate day-to-day activities, support patient care, and work with management to build the team and improve service delivery.

Qualifications

• 3+ years of administrative and/or community-based experience in medical/clinic settings or community organizations.
• Experience supervising or leading a team is preferred.
• Strong ability to manage patient challenges, access resources, and make referrals.

Responsibilities

• Supervise day-to-day activities of a team of community health workers focused on Enhanced Care Management.
• Provide direct support in managing patient challenges and facilitating access to resources and referrals.
• Collaborate with management to build and develop the team, ensuring alignment with goals.
• Coordinate personnel administration, including hiring, training, and performance management.
• Provide administrative support to leadership, including scheduling, documentation, and reporting.
• Assist in leading staff meetings and attend leadership meetings.
