Job summary

Enhabit Home Health & Hospice in Alabama seeks a licensed registered nurse to oversee the coordination of home health services, ensuring care meets standards and company procedures.

You will collaborate with physicians, referral sources, patients, caregivers and field staff to maintain care coordination and continuity. In-office attendance is essential to supervise, support the team and ensure compliance with federal, state and local guidelines.

Qualifications

• Required graduate of an approved school of professional nursing.
• Must maintain a valid clinical nursing license per state requirements.
• Experience as a licensed professional may be deferred with VP/Regional President approval.
• Minimum two years nursing experience within the last 36 months.
• Minimum one year home health nursing experience.
• Knowledge of federal, state and local laws governing home care operations.
• CPR certification required.
• Valid state driver license and automobile liability insurance required.
• Basic demonstrated technology skills including mobile devices.
• Prior management experience and experience with Homecare Homebase (preferred).
• Bilingual English/Spanish (preferred) to aid communication.

Responsibilities

• Coordinate home health services delivered to patients according to standards and procedures.
• Review and approve patient information during start of care, recertification, resumption of care or evaluation visit.
• Facilitate relationships between physicians, referral sources, patients, caregivers and employees.
• Ensure scheduling of patients to field clinicians and communicate with staff and providers to maintain care coordination.
