Job summary

Enhabit Home Health & Hospice seeks a licensed registered nurse to coordinate in-home care, ensuring care plans meet standards and patient needs are met.

You will collaborate with physicians, patients, caregivers and field staff to schedule visits and maintain care continuity. In-office attendance is essential to supervise and support the team; CPR, a valid state license, and home health experience are required or preferred.

Qualifications

• Required graduate of an approved school of professional nursing.
• Valid clinical license per state requirements or compact license.
• Minimum two years of nursing experience within the last 36 months.
• Minimum one year of home health experience.
• Knowledge of federal, state and local laws governing home care operations.
• CPR certification required; valid state driver license.
• Automobile liability insurance as required by law.
• Basic technology skills including mobile device operation.
• Preferred management experience.
• Preferred Homecare Homebase (HCHB) experience.
• Preferred bilingual English/Spanish communication.

Responsibilities

• Coordinate home health services to patients according to standards and procedures.
• Review and approve patient information at start of care, recertification, resumption of care, or evaluation visit.
• Facilitate relationships between physicians, referral sources, patients, caregivers, and employees.
• Schedule patients to field clinicians and maintain care coordination and continuity with staff and providers.
