Job summary

Enhabit Home Health & Hospice is seeking an experienced registered nurse to coordinate home health services, collaborate with physicians and caregivers, and ensure high-quality patient care. The role requires 2+ years of nursing experience in the last 36 months and 1 year in home health, CPR, and a valid license.

On-site position with essential in-office attendance to supervise teams, drive care excellence, and support regulatory compliance. Leadership preferred.

Qualifications

• Graduate of an approved school of professional nursing.
• Valid RN license per state requirements (or compact license).
• Minimum two years of nursing experience in the last 36 months.
• Minimum one year of home health experience.
• Knowledge of federal, state and local home care laws and regulatory guidelines.
• CPR certification.
• Valid state driver license and automobile liability insurance.
• Basic technology skills, including mobile devices and MS Office.

Responsibilities

• Coordinate the overall home health services delivered to each patient per standards and procedures.
• Review and approve patient information during start of care, recertification, resumption of care, or evaluation visits.
• Facilitate the relationship among physicians, referral sources, patients, caregivers and staff.
• Ensure scheduling of patients to field clinicians and maintain care coordination and continuity of care.
