The role requires RN licensure, CPR, and at least two years of nursing experience with one year in home health. A valid driver’s license and solid technology skills are essential for success in this on-site role.

Qualifikationen

• Graduate of an approved school of professional nursing.
• Active RN license with CPR certification.
• Minimum two years of nursing experience in the prior 36 months; at least one year in home health.
• Valid state driver's license and automobile liability insurance.
• Strong communication skills and basic technology proficiency.

Aufgaben

• Coordinate overall home health services to meet standards of practice and company procedures.
• Review patient information during start of care, recertification, resumption of care, or evaluation visits.
• Facilitate relationships among physicians, referral sources, patients, caregivers, and staff.
• Schedule patients to field clinicians and maintain care coordination and continuity.
