Job summary

Enhabit Home Health & Hospice in Montgomery, AL is seeking a skilled registered nurse to oversee home health services, ensure clinical standards, and coordinate care delivery. You will work with physicians, patients, and caregivers, handle start-of-care and recertifications, and ensure scheduling and communication across field staff to maintain seamless continuity of care.

The ideal candidate has a nursing license, 2+ years of recent nursing experience (including home health), CPR certification,

Qualifications

• Graduate of an approved school of professional nursing.
• Valid clinical nursing license per state requirements.
• Minimum two years nursing experience within last 36 months.
• At least one year in home health.
• Knowledge of federal/state/local laws governing home health.
• CPR certification.
• Valid state driver's license and automobile liability insurance.
• Strong technology skills including mobile devices.
• Preferred management experience.
• Preferred Homecare Homebase (HCHB) experience.
• Preferred bilingual English/Spanish.

Responsibilities

• Coordinate overall home health services per standards.
• Review and approve patient information at start of care, recertification, resumption, or evaluation.
• Facilitate relationships between physicians, referral sources, patients, caregivers, and staff.
• Schedule patients to field clinicians and maintain care continuity.
