Enhabit Home Health & Hospice in Virginia seeks a registered nurse to coordinate and oversee high-quality home health services for patients. You will collaborate with physicians, caregivers and staff to ensure effective care planning and timely patient support.

The role emphasizes adherence to federal, state and local guidelines, scheduling, and ensuring optimal patient outcomes. In-office presence is essential to supervise teams and maintain service quality.

Formación

• Graduate of an approved school of professional nursing (RN).
• Must maintain a valid clinical license per state rules, or compact license.
• Minimum 2 years nursing experience within the last 36 months.
• Minimum 1 year experience in home health.
• Knowledge of federal, state and local home care regulations.
• CPR certification and valid state driver's license.
• Automobile liability insurance as required by law.
• Basic technology skills, including mobile devices.

Responsabilidades

• Coordinate home health services delivered to patients per standards and procedures.
• Review and approve patient information at start of care, recertification, resumption, or evaluation visits.
• Facilitate communication between physicians, referral sources, patients, caregivers and staff.
• Schedule patients to field clinicians and ensure care coordination and continuity.
