HELP USA is seeking a Clinical Case Manager Supervisor to oversee clinical case management staff and interns. You will supervise and guide services for 111 families with young adults 18–25, focusing on housing stability and access to community resources.

Responsibilities include chart audits, service planning, and coordinating with medical, mental health, and substance use providers. Strong documentation and collaboration skills are essential.

Competenze

• Master of Social Work or related field required.
• Minimum of 3+ years of experience in case management and supervision.
• Experience with NYC benefits programs and housing subsidy resources is a plus.

Mansioni

• Provide weekly supervision to case managers and interns.
• Audit charts and files regularly.
• Assess cases, formulate service plans, and connect tenants to housing resources.
• Facilitate support groups and liaise with medical and behavioral health services.
• Document case information in AWARDS and prepare reports.
