Job summary

Little Flower Children and Family Services of New York is seeking a Case Planner Supervisor in Brooklyn, NY. Under the general supervision of a Director, you will oversee a small unit of up to four case planners, providing guidance, training, and ensuring compliant, timely casework for foster care families.

You will assign and review cases, develop action plans, monitor permanency timelines, and serve as a liaison with internal staff and external partners while maintaining documentation

Qualifications

• Thorough knowledge of the principles, practices, and techniques of case management.
• Good knowledge of agency rules and regulations relating to the program.
• Ability to plan, assign, and supervise the activities of a casework unit.
• Ability to analyze and evaluate case records and guide staff decisions.
• Ability to prepare clear, accurate, and comprehensive reports of unit activities.
• Excellent interpersonal and mediation skills.

Responsibilities

• Supervises a unit of up to 4 case planners and conducts conferences to guide, train, and develop casework skills.
• Evaluates staff performance, reviews case records, and maintains appropriate records and controls.
• Conduct structured supervision around compliance and permanency timelines to provide feedback and motivation.
• Resolve workers’ concerns by creating corrective action plans or developmental plans when necessary.
• Act as a liaison between internal staff and external organizations to coordinate case plans.
• Ensure coordination of all specialists’ contacts into the case plan and foster teamwork.
• Maintain a time-oriented approach to documentation including progress notes, psychosocials, and FASPs.
• Create clear job expectations for staff and supervise weekly performance.
