Job summary

Little Flower Children and Family Services of New York is seeking a Case Planner for the TFBH program in Wading River, NY. Little Flower Children and Family Services of New York is a human services nonprofit supporting the well-being of children, youth, families, and individuals across New York City and Long Island since 1929. Our staff of more than 500 builds well-being by providing prevention services, foster care, residential treatment care, adoption services, programs and services for individuals with developmental disabilities, and medical and mental health services.

Principle Responsibilities
  • Obtains data regarding child’s and family’s social and medical history. Provides case planning.
  • Advocates for and obtains appropriate services for the child, including educational, clinical, and recreational services.
  • Meets regularly with foster parents to clarify issues, provide and/or obtain concrete and supportive services. Monitors the quality of care provided to the child. Keeps foster parents advised of child’s condition and prognosis.
  • Engages the child’s family in casework and group work activities aimed at sustaining relationships through visiting. If possible, works towards family reunification. Assist with adoption related activities if necessary.
  • Arranges for regular training of foster parents.
  • Share 24-hour 7-day emergency cell phone coverage with the Behavior Specialist. Responds to emergencies after hours or on weekends.
  • Performs related duties as required.
Required Qualifications and Experience
  • MSW; M.A. + 1 year experience; B.A. + 3 years related experience; or B.A. + 5 years unrelated experience.
Travel Requirements

This position offers a full-time, 35 hours per week, hybrid/in field schedule in Wading River, NY. It requires routine travel throughout Suffolk and Nassau County.
