Job summary

Heartshare in New York City is seeking a Floating Case Planner to assist case planners with day-to-day social services tasks for children in foster care. You will partner with families to develop safety and permanency plans, coordinate mandated visits, and support families with access to services to achieve child well-being.

Responsibilities include attending Family Court hearings, participating in Family Team Conferences, and collaborating with medical, mental health, and case management teams

Qualifications

• Bachelor's degree required; knowledge of Child Welfare systems preferred.
• Excellent written and verbal communication.
• Strong collaboration with multidisciplinary teams.

Responsibilities

• Create service plans addressing safety, behavioral, educational and permanency needs in partnership with families.
• Monitor child safety and progress; document in case notes.
• Schedule and supervise mandated visits; conduct interviews in homes, schools, and other sites.
• Attend supervision meetings and Family Court hearings; provide testimony as needed.
• Participate in Family Team Conferences and coordinate with Integrated Services to meet medical, mental health, and welfare needs.
• Serve as alternate support for FTCs and capture data to measure outcomes.
