Job summary

SCO Family of Services is seeking a Case Planner in New York City to support safety, permanency, and well-being for children and youth. You will work with families to implement service plans, address barriers, and coordinate with schools, medical and mental health providers.

The role requires collaboration with foster parents and birth parents, core case planning, TRIPP training, and active participation in hearings and court appearances as needed.

Qualifications

• Bachelor's degree required; BSW/MSW preferred.
• Background clearances and fingerprinting required.
• Excellent oral and written communication.
• Proficiency in Microsoft Office, Excel and Connections.
• Bilingual abilities encouraged.

Responsibilities

• Complete initial face-to-face contact with the child within 2 business days.
• Conduct ongoing face-to-face contacts with the child, parent and foster parent on caseload.
• Complete safety checklists and risk assessments on a monthly basis.
• Count and monitor medication and medication compliance.
• Document in Connections within 7 business days of the event date.
• Assess safety and risk and develop safety plans with families.
• Collaborate with families to engage in service plan development and assessment.
• Make referrals for services (Health Homes Care Management, CFTSS, Preventive, therapy).
• Support foster and birth parents and accompany them to appointments.
• Attend trainings and required meetings; participate in court appearances as needed.
