Job summary

Peter & Paul Community Services seeks a Lead Case Manager in Saint Louis to provide leadership of the Rally Veterans Housing Program, oversee day-to-day case management, ensure VA GPD compliance, and coordinate services for veterans.

This role works from a Housing First, trauma-informed perspective, collaborates with VA partners and healthcare providers, manages staff, and supports residents toward housing stability and recovery.

Qualifications

• Bachelor’s degree in Social Work, Human Services, Psychology, Sociology, Criminal Justice, or a related field; equivalent education and experience may be considered.
• Minimum of two years of case management or supportive services experience with vulnerable populations.
• Experience coordinating programs, leading staff, or providing day-to-day direction in residential or housing settings.
• Knowledge of Housing First, trauma-informed care, motivational interviewing, and harm reduction.

Responsibilities

• Provide day-to-day leadership and oversight of the Veterans Housing Program.
• Maintain a caseload of 15 Veteran residents and provide individualized case management in line with VA GPD requirements.
• Complete program intakes, assessments, service plans, progress notes, and discharge summaries within timelines.
• Coordinate admissions, discharges, and ongoing communication with VA HCHV Liaison and partners.
• Supervise and coach Program Safety Monitors to ensure consistent resident engagement and documentation.
• Respond to crises after-hours as needed.
• Collaborate with VA, healthcare providers, housing agencies, and community organizations to support housing stability and well-being.
